Enrollment intake and informational booths will be available!
Documents needed:
• Tribal enrollment (child's letter or parent/guarrdian Tribal I.D.)
• Income (Work/Per-cap Pay Stub, TANIF, Letter, ETC.)
• Immunization record (GRHC will be onsite to assist)
• Other documents: Court docs. (if needed), Snap Letter (supplement nutrition assistance program)
